PARTICIPANT WAIVER, TERMS & CONDITIONS, MEDICAL CONSENT AND RELEASE OF LIABILITY

PARTICIPATION, RISK & SAFETY
I understand that participation in basketball training, games, camps, clinics, conditioning activities, and related programs involves inherent risks including falls, collisions, physical contact, equipment-related incidents, serious injury, illness, permanent disability, property damage, and death.

I voluntarily accept and assume all risks associated with participation.

I agree that the participant will follow all academy rules, safety requirements, and coach instructions at all times.

Participation is voluntary and undertaken at the participant’s own risk.

MEDICAL CONSENT & HEALTH RESPONSIBILITY
I authorize Tiger Basketball Academy Ltd. to provide basic first aid, contact emergency services, arrange ambulance transportation, and obtain emergency medical treatment when deemed necessary.

I agree to disclose any medical conditions, allergies, injuries, medications, or health concerns that may affect participation.

I understand that Tiger Basketball Academy Ltd. may require medical clearance before allowing a participant to return following an injury, illness, or concussion.

I understand and agree that Tiger Basketball Academy Ltd. does not provide accident insurance, medical insurance, physiotherapy coverage, rehabilitation coverage, or healthcare benefits of any kind.

All medical expenses, including ambulance fees, hospital treatment, physiotherapy, chiropractic treatment, rehabilitation services, prescription medication, specialist consultations, and related healthcare costs shall be solely the responsibility of the parent/guardian.

PROGRAM POLICIES
Tiger Basketball Academy reserves the right to modify schedules, locations, facilities, coaches, programs, and training formats when necessary.

Registration and tuition fees reserve a participant’s place in the program and are not based solely on attendance.

Missed classes, absences, vacations, illness, transportation issues, scheduling conflicts, weather concerns, or personal circumstances do not automatically qualify for refunds, credits, or make-up classes.

The academy reserves the right to suspend or remove any participant whose behaviour is unsafe, disruptive, abusive, threatening, or contrary to academy policies.

Parents are responsible for timely drop-off and pick-up of participants.

The academy is not responsible for lost, stolen, or damaged personal property.

MEDIA RELEASE & PROPERTY RESPONSIBILITY
To the fullest extent permitted by law, I release, waive, discharge, and hold harmless Tiger Basketball Academy Ltd., its owners, directors, officers, coaches, employees, contractors, volunteers, facility providers, school districts, municipalities, landlords, agents, and representatives from any and all claims, demands, actions, damages, losses, liabilities, costs, expenses, or legal fees arising directly or indirectly from participation in academy activities or use of facilities.

I agree to indemnify and hold harmless Tiger Basketball Academy Ltd. from any claim, demand, action, liability, damage, cost, expense, or legal fee brought by or on behalf of the participant, parent/guardian, family members, heirs, executors, administrators, representatives, or any third party arising from participation in academy activities.

This Agreement shall be governed by the laws of British Columbia and Canada. If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.

RELEASE OF LIABILITY, INDEMNITY & AGREEMENT
Any damage caused to facilities or equipment through unauthorized use or use without supervision will be the financial responsibility of the participant.

ACKNOWLEDGEMENT
By signing below, I confirm that I have carefully read and fully understand this entire Agreement. I voluntarily accept all risks of participation, authorize emergency medical treatment, accept responsibility for all medical and healthcare-related expenses, consent to the media release contained herein, and agree to all terms and conditions of this Agreement.
